David Sinclair vs. Pfizer: Who Was Right?

David Sinclair has spent much of his career arguing that aging is not necessarily permanent. Today, he is best known for partial cellular reprogramming, but an earlier controversy involving resveratrol, SIRT1, and Pfizer nearly derailed his career.

After examining the original research, I believe Pfizer found a genuine flaw—but Sinclair later showed that it was not the whole story.

In 2003, Sinclair and his colleagues reported that resveratrol, a compound found in grapes and red wine, activated the protein Sir2 and extended lifespan in yeast. Humans possess a related protein called SIRT1, which helps regulate metabolism, stress responses, DNA repair, and other cellular processes.

The possibility of activating these protective systems with a drug attracted enormous interest. Sinclair helped establish Sirtris Pharmaceuticals, which developed resveratrol formulations and more powerful synthetic compounds. GlaxoSmithKline purchased Sirtris for approximately $720 million in 2008.

Then scientists found a problem with the original laboratory test.

The assay measured SIRT1 activity using an artificial peptide carrying a fluorescent chemical tag. Researchers discovered that resveratrol appeared to activate SIRT1 when the tag was present but not with several untagged substrates. This suggested that the compound might be interacting with the test rather than generally activating SIRT1.

In 2010, Pfizer scientists strengthened that challenge. They reported that resveratrol and three Sirtris compounds did not directly activate SIRT1 with the native substrates they tested. They also found off-target effects. In one mouse experiment, the Sirtris compound SRT1720 failed to reduce blood glucose or improve mitochondrial capacity as previously expected.

My interest in this dispute has a personal dimension. In the late 1990s, I worked in distribution for a generic pharmaceutical company and later performed temporary work for Pfizer. I was not a scientist and had no role in developing or approving drugs, but we followed drug information closely and heard industry discussions the general public rarely encountered.

I remember conversations about human trials, patents, profits, and legal exposure. One person in distribution told me that lawsuits amounted to “fractions of a penny” compared with the money involved.

That was one person’s informal remark as I recall it decades later—not an official Pfizer statement. I remember it because it revealed how financial risk could be viewed inside an enormous industry.

My experience did not convince me that pharmaceutical companies are inherently evil. It taught me that lifesaving science, patient safety, commercial ambition, and legal risk can exist within the same organization. I therefore do not assume a corporation is correct merely because it has more money, laboratories, attorneys, or public-relations power. The evidence must decide the question.

Sinclair returned to the laboratory. In 2013, his group published evidence that resveratrol and related compounds could activate SIRT1 when particular natural features were present in the substrate. His team also identified one amino acid in SIRT1 that was essential to this effect. Altering it prevented activation, supporting a direct but substrate-dependent mechanism.

The study included GSK researchers and authors connected to the commercial development of SIRT1 activators, so it should not be treated as completely independent vindication. More importantly, an independent German group tested 6,802 physiological peptide sequences. It found that resveratrol increased SIRT1 activity on a small subset, inhibited another subset, and produced little change in most. The researchers also confirmed activation on an untagged peptide using mass spectrometry.

So, who was right?

Pfizer was correct that the original fluorescent assay could produce misleading results. Resveratrol is not a universal switch that simply turns SIRT1 on, and some early claims were too broad.

Sinclair was correct that the mechanism was not entirely an experimental illusion. His group and independent researchers produced evidence that resveratrol can directly influence SIRT1 with particular biological substrates.

Sinclair therefore recovered an important part of his argument, but he did not prove that resveratrol extends human lifespan or reverses human aging. No human trial has established either outcome.

For years, Sinclair has publicly said that he personally takes approximately one gram of resveratrol, commonly mixed with yogurt to improve absorption. That tells us he remains persuaded enough to experiment on himself, but one scientist’s personal routine is not clinical evidence or medical advice. Sinclair has generally presented his supplement choices as personal decisions made while the science continues developing.

I am on Team Sinclair, but supporting a scientist should not require pretending every early claim was correct. I respect him because he faced a potentially career-ending challenge, returned to the laboratory, and developed a narrower and more sophisticated explanation.

His current work on partial cellular reprogramming is separate from resveratrol. ER-100, based on controlled expression of three reprogramming factors, is now in a Phase 1 trial for glaucoma and another optic-nerve disorder. The trial is primarily testing safety—not proving whole-body age reversal—but it represents a remarkable transition from animal research into humans.

The Pfizer dispute was not a story about a heroic scientist defeating an evil corporation. Pfizer identified a real weakness. Sinclair showed that the weakness did not erase the entire mechanism.

Some questions were answered. Others remain open. Resveratrol may ultimately prove useful for particular conditions, particular people, or particular SIRT1 targets without ever becoming a universal longevity drug.

That uncertainty is not a failure of science. It is where science begins.
